Acute-on-chronic subdural hematomas (SDHs) are usually seen following head injuries and are frequently seen in elderly patients.1 Due to repeated bleeding, subdural clot during resolution forms membrane, which, because of its osmotic properties, starts expanding. Repeated bleed and expansion of chronic SDH can lead to cerebral herniation.
